There is annual maintenance that has to be done on tankless water heaters due to the design and how they work.  A tankless water heater reduces the pipe size inside to slow the water down so that it can "instantaneously" heat it.  This causes scaling due to the minerals in the water that would in a tank type heater settle to the bottom of the tank to stick to the sides of the piping inside the tankless water heater.  After a while that small pipe continues to get smaller, that is why every year they need to be de-scaled and the filters cleaned on them.  Without this they would quit working.
